Laporan Kasus Post Histerektomi Subtotal atas Indikasi Plasenta Akreta

Plasenta akreta merupakan implantasi abnormal plasenta pada dinding uterus. Perempuan yang paling berisiko mengalami plasenta akreta adalah mereka yang telah mempunyai kerusakan miometrium yang disebabkan oleh operasi sesar sebelumnya dengan plasenta previa. Rata-rata pasien dengan plasenta akreta tidak menunjukkan gejala. Temuan ini sebagian besar terlihat pada kasus dengan plasenta previa, yang merupakan faktor risiko terkuat untuk plasenta akreta. Perempuan 45 tahun datang dengan keluhan perdarahan pada jalan lahir sejak 3 hari sebelum masuk rumah sakit. Darah berwarna merah segar. Terdapat riwayat SC 2 kali pada anak kedua dan ketiga atas indikasi plasenta previa totalis. Pasien didiagnosis P3A0 dengan perdrahan postpartum et causa plasenta akreta dengan rupture ureter, anemia ec HPP, hipoalbumin dan hiponatremia. Pasien ditatalaksana dengan pemantauan tanda-tanda vital ibu, transfusi darah dan histerektomi subtotal.
