Paraplegia Inferior Tipe UMN sebagai Manifestasi Klinis pada Pasien Spondilitis dengan Penyebab Infeksi Nonspesifik

Spondilitis dengan penyebab infeksi nonspesifik adalah infeksi destruktif pada tulang belakang atau struktur paraspinal oleh karena agen infeksi nonspesifik. Perkembangan penyakit spondilitis dapat mengakibatkan destruksi dari vertebra dan meluasnya inflamasi hingga ke ruang epidural dan paraspinal. Hal ini dapat menyebabkan kerusakan tulang belakang dan komplikasi neurologis lainnya salah satunya paraplegia inferior. Berikut ditampilkan laporan kasus mengenai laki-laki berusia 56 tahun dengan gejala dan tanda klinis dan defisit neurologis yang meliputi nyeri punggung bawah dengan awitan gradual, paraplegia inferior tipe upper motor neuron (UMN) dengan awitan gradual progresif, dan parahipoestesia dan parahipoalgesia setinggi segmen myelum thorakal 7. Pada tahap awal evaluasi penyakit, gejala dan tanda klinis tersebut mengarah pada kemungkinan suatu diagnosis tumor myelum dengan diagnosis banding spondilitis tuberkulosis. Hasil biopsi jaringan tulang yang diambil saat operasi, ternyata menunjukkan adanya peradangan kronik yang mendukung pada diagnosis spondilitis infeksi dengan penyebab nonspesifik.Downloads
