Anemia hemolitik autoimun (Autoimmune Hemolytic Anemia = AIHA / AHA) adalah sebuah kelainan pada eritrosit yang ditandai dengan kerusakan eritrosit oleh autoantibodi dalam tubuh seseorang. AIHA dapat terjadi pada semua kalangan usia, namun paling sering menyerang usia di atas 40 tahun. Angka kejadiannya sebesar 17:100.000 orang per tahun dengan angka kematian hingga 11%. Secara umum AIHA terjadi secara intravaskuler dan ekstravaskuler dengan aktivasi komplemen atau tanpa aktivasi komplemen. Pada pasien AIHA biasanya mengalami pucat, lemas, dispnea, hipotensi, takikardi, ikterus pada mukosa kulit, splenomegali, dan hepatomegali. Untuk menegakkan diagnosis AIHA selain dari anamnesis dan pemeriksaan fisik, juga didapatkan dari pemeriksaan laboratorium yaitu berupa pemeriksaan darah lengkap, indeks eritrosit, LDH, bilirubin indirek, retikulosis, serta morfologi darah tepi. Selain itu perlu dilakukan juga pemeriksaan untuk mendeteksi autoantibodi pada eritrosit. Pemeriksaan ini biasa dikenal sebagai Coombs test yaitu berupa direct Coombs test (Direct Antiglobulin Test/DAT) dan indirect Coombs test (Indirect Antiglobulin Test/IAT).Downloads
