PENENTUAN CUT-OFF INDEKS DISKRIMINASI DIAGNOSIS PADA ANEMIA DEFISIENSI BESI DAN BETA THALASSEMIA MINOR

Beta thalassemia minor (BTM) dan anemia defisiensi besi (ADB) merupakan dua kasus yang paling sering ditemui pada anemia mikrositik hipokoromik ringan. Diagnosis banding antara ADB dan BTM menjadi perhatian penting bagi para dokter untuk menghindari terapi zat besi yang tidak perlu dan untuk menghindari diagnosis BTM yang salah. Oleh karena itu, diperlukan formula diferensiasi yang cukup valid dan mudah dilakukan. Salah satunya dengan menggunakan indeks red blood cell (RBC) pada pemeriksaan hematologi dasar. Penelitian ini merupakan penelitian potong lintang dengan 30 sampel ADB dan 30 sampel BTM di RSUP DR. Sarjito Yogyakarta. Anemia defisiensi besi didefinisikan sebagai anemia mikrositik, yaitu mean corpuscular volume (MCV) <80 fL dengan kadar feritin <12 μg/dl dan hemoglobin A2 >3,5%. Beta thalasemia minor didefinisikan sebagai mikrositosis dengan hemoglobin A2 ≤3,5%. Kami membandingkan sepuluh formula diskriminasi yang berbeda dengan tujuh parameter sel darah merah. Penghitungan meliputi nilai cut-off, sensitivitas, spesifisitas, nilai prediktif positif (NPP), nilai prediktif negatif (NPN), dan evaluasi kurva reciever operative characteristic (ROC). Hasil dari penelitian ini menunjukkan bahwa formula Green & King, RDW index dan Sirdah memiliki area under curve (AUC) yang terbesar, masing-masing 0,995; 0,992 dan 0,0909 dengan nilai cut-off masing-masing sebesar <84,7; <240,7; dan <40,12. Indeks formula Green & King, RDW index dan Sirdah merupakan indeks formula yang tertinggi untuk membedakan BMT dari ADB pada populasi RSUP DR. Sardjito.Downloads
